• You are aged between 9 and 65
• You are an Australian citizen, permanent resident or protected special category visa holder
• You live in Australia
• You have a disability caused by a permanent impairment
• You usually need disability specific support to complete daily life activities

If you answered yes to all of these criteria then you may be eligible to access the NDIS and can either make a verbal application by phoning the NDIS or a written application by completing an access request form
